FRM 40508 Oracle error

If there aren't any records found that match the last name and if I do a commit, the record gets inserted 2x. If there is a match then I receive the FRM 40508 error when trying to insert.

The table does not have any unique indexes. All fields that are required are populated. When I try inserting the same information manually in SQL Plus the record is inserted without a problem.

Any ideas on what might be wrong?
