This question comes up often on here it seems. I've seen others recommend at least one of these, and I would certainly recommend them (and have before, even!)
Database Design for Mere Mortals by Michael J. Hernandez

This book takes a very practical approach to database design, and cuts a
little short on theory, but it's great to get a good idea of how a project
happens from thought to finish.
Access Database Design and Programming by O'Reilly -- www.oreilly.com
ISBN: 1-56592-626-9
This book has a thorough section on Database Design, and in my opinion
approaches it from theory rather than practical application. (Functional
Dependency! YAY!) It also has great info on Query Language and a little
Relational Algebra for fun. All this and lots of info on VBA.
There is also a quality Whitepaper by Microsoft, of course this is from a Microsoft Access point of view, but as with Relational Design, you can weed out the advertisements and apply it to anything.
