Generally this type of error occurs because of data type / data length mismatch. That is if in the declare section U have declared name varchar2(30)

and U have a record where the name is longer than 30 chars

Or if U declared
value number(5)

and U have a record where the VALUE is NOT AVAILABLE.

Check the data & the variables declared. Thanks
