On your SP5 machines did you previously have a SP3 installed? NT Service Packs should be cumulative but sometimes they aren't. Some changed DLLs with SP3 don't come with SP5 so you need to install SP3 before SP5. I had stability and performance problems on a machine that went from installation directly to SP5. After af fresh install, SP3 and then SP5 the performance and stability is so much better. I think the SP3/SP5 problem is a known issue with OAS 4.0.8.1.
