GPF encountered when starting DEV2000 FORMS 4.5

I recently installed Personal Developer 2000 and FORMS 4.5 from CD. The installation was clean and the ORACLE7 database started fine (green light). When launching DEV 2000 FORMS, however, I got "Application Error... F45DES caused a GPF in module TK21WIN.DLL at 001A:0B67. Has anyone else encountered this GPF? What's the fix?
