When Oracle Forms retrieves data from the db, it quite often buffers it on the local disk in temporary files.
It sounds like Oracle Forms is trying to create a temp file, and the OS is encountering an IO error(full device maybe). Doesn't sound like a problem with Oracle Forms. May want to check out your device.
