Help with Forms 4.0 Error

I am a Forms 4.0 novice user. Currently we have Form 4.0.11 running on SunOS 4 and Oracle 7.0.12. When running my form application, I got this error message:  

    FRM-40908: RAM Internal Error: IRM: file irmfwr.c: line 71: Disk I/O Failure  

This seems to occurs when my application trying to retrieve data from the database. However, the application proceeds as usual. When I exit the application, I got another message:  

    ERROR: -1 form processing allocations, -6321560 bytes, not freed.
