Make sure that you have run the root.sh and rootpre.sh scripts since installing the product. As a final check, make sure that async IO is enabled on your machine where the database is. Oh yeah, make sure that the 'oracle' executable is setuid.
BTW: 'smit aio' to check for async aio.

There is no such command (as far as I know) to do this. You need to judiciously examine the PTF's installed.
