In our shop, DBA's create all databases. Beyond that, the developer can create, modify and delete tables, indexes, views, etc. The developer has total authority to alter a database that was created for him/her.

The DBMS folks are available for consultation when people want ideas for data modelling and how to set up effective databases.

The DBA creates a test database from the production database. They can copy the current contents or allow the developers/testers to populate the tables.
