Choose Form block fields dynamically

I'm looking for functionality like the below...

The user would initially be provided with a form with 10 choices(check boxes) which are field names.

The user will select all the fields which he wants to see and submits. Now we should display a control block which contains fields to enter query criteria,and below that in another block should be displayed the fields we selected earlier.
