It's available through my institution in the ACM Digital Library, ACM SIGMOD Record Volume 7 Issue 3-4, 1975.

It's (obscurely) in the Source Materials / Front Matter / pdf download. Which is actually a scan of the whole issue, except for a few articles which have been excised and are available as the (only) articles listed in the electronic Table of Contents.
