No intent to diminish the rest of your post, but the word "implicit" caught my eye, which I object to, thinking as I do, that Codd had no implicit indices or indexes, however they should be spelled, in mine. But that paragraph is important in a big way for me, application people, who in those days constituted ALL OF THE PROGRAMMERS IN THE WORLD, had to arrange EVERYTHING. No wonder they were pre-occupied with hardware.   DBA's became a part of the bureaucratic "relief", if I may call it that, for this, but I say "too bad!".
