Forms 4.5 Menu ?

I build a menu and attach it to a form. When I run the form, the following
error message displays: FRM - 10221 - Cannot read file (name). When I
press
OK on this message, another error message appears: FRM 41810 Error
creating menu.
When I create the menu, I am able to generate it with no problems.
I attach the menu to the form and generate the form with no problems. The
errors occur only during runtime.
Any help would be greatly appreciated
