Hi,
We have had the same problem: the answer given by Oracle support is that you can't use the same orawin directory for Forms 4.0 and Forms 4.5, because of Required Support Files conflict. The solution is to separate F4.0 and F4.5 in 2 orawin directories. (Pb: you can't use them simultaneously).
