Have a look at the preface of the book "Systems Analysis and Design" by Shelly, Cashman and Adamski 2nd Edition (ISBN 0-87709-631-7) pg xvi about the CASE tool "Visible Analyst Workbench". It's available in both DOS and Windows. Haven't used or seen it myself though.
