Help: RME-00222 Failed to dispatch operation to Repository

When inserting in the Repository with one of the Designer/2000 products we often get the error:
RME-00222: Failed to dispatch operation to Repository The error is always followed by the error:
REME-00011: Operation 'ins' on XXX has failed When creating a new primary key XXX has the value UNIQUE_IDENTIFIER_ENTRY. When creating a new table usage within the Module Data Diagrammer XXX has the values MODULE_DETAIL_TABLE_USAGE and MODULE_TABLE_USAGE.
At first we did not have this error, but since yesterday the most insert actions fail.
