I had in an Oracle consultant 2 months ago who insisted on using Forms 4.0.13 for the following reasons:
1) More stable, 2) Portable to Mac and Unix platforms 3) No advantage to 4.5 except for the debugger and multi-media support (his words)

Forms 4.5 for Mac was promised in March & I'm still waiting for it. My 7.1.6 for UNIX shipped with Forms 3.0 (a "discontinued product") and forms 4.0 - I received that package at the end of July.
Oh, with Oracle products make sure you keep track of the version number to the third place, i.e. Forms 4.5.5 vs 4.5.6 it can be a big difference.
