I don't know about Forms 4.6, but the latest version of Oracle Forms for Windows comes in the Developer 2000 package, which is 4.5.6.

As for Forms 5.0, I noticed a couple of presentations on what it will be like in the abstract listing for Int. Oracle User's Week 1995. I've included them here.
