Oracle, at least in its DOS tools, used to provide a program called

DB3Prep that did it all - Built the Oracle table, wrote the CTL file and loaded the data... I don't know if it is still around in the newest tool set, but you might try to find it. Otherwise, Approach for Windows will work.
