Apparently SQL*Forms is unable to find a terminal definition which matches the name of your term enviorment variable. You can overide the env variable by using a command line switch. Try using sqlforms30 term=ansi
Substitute your term type for ansi. Oracle provides a bunch of termdefs with different default characteristics. By renaming one of them, you system administrator set up the default terminal type for your site.
