Hi !
I have upgraded to 4.0.13 some month ago because I had major problems with referencing objects in SQL*Forms 4.0.12.
The problem was, that after a while of building my form with a lot of references to a stored template form, I couldn't open the form anymore. So I was completly upset and called our local oracle support. They told me that oracle strongly recomends, to not use references in 4.0.12 and that in 4.0.13 most of the bugs are solved. But they aren't. It's much better but I had still some cases of corupted forms........
