ORA-1578 why?

We are running Oracle version 6.0.36 on an HP T-500 computer running HP-UX 9.04.
Occasionally we receive an Oracle 1578 error. This prevents us from running transactions which need to access the entire table. In order to "recover" access to the entire table we use "alter session set event ...", but we then lose a "blocksize" of data.
We have sent trace files to our Oracle representative, but did not receive a response.
The problem usually appears in an INDEX or rollback segment.
The disks where the .dbf files resided were functional and error-free.
- Why does Oracle error 1578 occur?
- Can it be prevented?
