Common problem. When you install Forms you give it its' own <OracleHome> directory. That <OracleHome> has its own NETWORK\ADMIN directory (or NET8\ADMIN) directory. When you run forms it uses the TNSNAMES.ORA file in that path, not in the path used by the tools that were installed with the server. Find all the TNSNAMES.ORA files (except the ones in SAMPLE directories) and make sure they are all the same.
