You need the Forms Server and Reports Server.
Read the following documents on MetaLink for details on how to
install/configure them:
Installing and deploying Forms/Reports 6i and WebDB listener on Windows NT

White Paper: Oracle Forms Server FAQ
You may want to use 6i instead of 6.0. It will still run your 6.0 FMX's without needing to re-compile them, and 6i is supposed to have cleaned up alot of the web errors that were in 6.0.
You are fine using Oracle 8.0.5, since it is compatible with Dev 6.
Last, you do not need the Forms Server and the database on the same box. It is standard (and preferred) practice to keep them on separate boxes.
