Please help: FRM-10256 - User is not authorized to run Form Builder Menu

When I started application I have got message:
FRM-10256 - User is not authorized to run Form Builder Menu Cause: You are not enrolled in Form Builder. You do not have SELECT permission on the Form Builder base tables. Action: Notify your DBA.
- Application works fine against other databases.
- First initial form reads menu (from file).
- Message comes prior entering to the PRE-FORM trigger or any other application code.
- We have 'Use security' property in menu set to Yes and have some roles used in menu.
- If I remove menu or disable security in menu it works fine.
- Setting of roles (in user_role_privs) is the same like in other databases.

What does mean the "Form Builder base tables" in error description ? Or what does it mean "be enrolled in Form Builder" ? It seems like it cannot see some system table.
