FILE SENDING TROUBLE !!

Hello,
I have to send a file trough WEB form. I had never done it and so I followed step by step the instructions of the Oracle's Manual:

1. I have made the form as the manual suggests;
2. I have created the DAD ;
3. The username associated with the DAD is granted the OWA_STANDARD_ROLE role in the database;
4. I have rewrited the simple procedure descibed by the manual (WRITE_INFO);

Until now all is OK.  

If I don't specify the field "file", all seems working right (but the file is not sended) instead if I specify the field after few seconds this message appears:

 "An application error has occured. Please try again"

I think I have done all as it is described.

PLEASE, HELP ME!! Here the specifics of the product components installed:

 CORE                       4.0.5.0.0        Production
 NLSRTL                     3.3.2.0.0        Production
 Oracle8 Enterprise Edition 8.0.5.1.0        Production
 PL/SQL                     8.0.5.1.0        Production

 TNS for 32-bit Windows: 8.0.5.0.0
