I know only two products that can control Form source code. The first one is PVCS Version Manager. You could find more information at http://www.intersolv.com/pvcs/. The other one is ClearCase. Look at
http://www.rational.com/products/clearcase/index.jtmpl for more information. We doesn't use one of them, well not yet, so I can't tell you if they're great or bad.
