Toolbar and disappearing icon image - Forms 4.5

Running forms on Apps 10.7 the following problem occurs (ocassionally)...

The standard toolbar will 'lose' the images and all the user can see are the gray
icons. The bubble text still appears and the icons are enabled.

Is this a known bug?
