Help! Pb. with Forms5 by Web

We would like to access Oracle Forms (5.0) by the Web, using the Developper 2000 Server on Windows NT 4.

This works fine... sometimes... both with the Oracle Application Server and the Microsoft Internet Information Server, but :

1. from time to time, we just cannot access the forms, and get an error message in an Java applet window, saying : "FRM-99999: Failed to connect to the Server: my_server:9000"
2. we first have to start the F50SRV32.EXE by hand (no way to get it start alone as a service or by any kind of startup script);

Are there any known bugs about these issues ?

Could someone help us to solve these problems ?
