We run a large heavily modified MRP II package that was written in Forms 3.0. We had to add a few pro*c programs for reporting, for performing some heavy duty batch update processing, and for transferring data back and forth to a legacy machine, but Forms works fairly well.
Since version 3.0 is unsupported I would not be using it for anything new. We have since upgraded/converted all our Forms to Forms 4.5, but in its day 3.0 was a pretty good product.
