Forms 4.5 cant's connect to database

When I try to connect to Personal Oracle 8.0 from Forms 4.5, it says

ORA-09354: Windows NT Two-task driver: Oracle task unexpectedly died.

I can connect to database (on the same machine, which is Win95) from sqlplus ONLY if I DON'T give host string, otherwise it gives 'listener couldn't resolve name...'.

Any help will be greatly appreciated.
