Adding List Elements to a Forms 4.5 List

Has anyone written a Forms 4.5 form that performs the example on page 9-19 of the Oracle Developer 2000 Forms 4.5 Developer's guide Manual? The example program creates a two column query record group and then populates a list item with the values from the record group.

I tried using using this example on a similar problem, but the list was not created. No errors were generated by Forms at run time, but something must be set wrong (one of the properties of the List?).
If anyone has Forms 4.5 source for this example or a similar type of list, I would appreciate seeing a complete example. Thanks...
