It really is quite straightforward, with the caveats as id'd in the book (no MDI, etc) and as outlined below. The process involves:

  1. Given an fmb, one must generate an fmx on the web server platform
  2. (assuming static HTML setup), create an HTML file which references the gen'd file, supplying a name and password as well (see the example in ORACLE_HOME/forms45/html)
  3. with the web server and forms server running, enter the URL for the HTML doc from above. This will cause a java applet to be downloaded and executed on your client machine. Depending on your local CPU, this may take between 2 and 15 minutes!
  4. Once loaded, again, depending on your local CPU, the resulting form will perform at a rate somewhere between "tolerable" and "totally unacceptable".

I spent several weeks with this env (using Solaris 2.5.1, netscape 3.0, and Dev/2K 1.6), and
have since ditched it, given (3) and (4) in our env. Hope your mileage is better!
