The forms message manual says

Cause
You tried to specify a value for the default value property or other values property that exceeds the value of the maximum length property. Action
Specify a value that does not exceed the value of the maximum length property.

My guess is the default value on the dates field and/or the values on check boxes / radio items may be causing this error.
