Forms 4.0 for HP-UX - relinking for user exits

I am attempting to relink the Forms runtime executables to add user exits.

Forms 4.0, HP-UX 9.07, Motif1.2, X11R5

When relinking any executable (even without user exits) using the sqlforms40.mk file the link fails. Altering the XLIBS to to use the /usr/lib/X11R5 directory and MOTIFLIBS to us the Motif1.2 directory, the link is successful, however every time I try to run the form, it crashes with a segmentation fault once the form name and connection dialog box is completed.

Has anybody experienced this problem before? What can be done to troubleshoot and/or correct the problem?
