Oracle provides an upgrade facility with Forms 4.5. Look at the options on the F45GEN command. Look closely at the keyword UPGRADE. F45GEN will upgrade a form from 3.0 to 4.5.
f45gen module=myform userid=scott/tiger upgrade=yes version=23 add_triggers=YES
