Forms Designer weirdness

I'm running d2k 1.3 Forms Designer (4.5.7) on NT 3.5.1 and am having the following weirdness...
double click on the "Forms Designer" icon, get the splash screen, the initial window flashes up and disappears, the splash screen hangs on for a bit and then goes away. No process is left running, no error message. Generate, Reports, Graphics, ... all run fine.

I've checked:

1. File permissions-- OK
2. Temp file-- OK
3. Registry-- OK
