A little, but important correction:

Forms 4.5.7.0.10 was part of Developer/2000 1.3 (the first release for Windows95 & NT) and this release was *not* certified against NT 4.0.

The NT 4.0 certification first emerged with Developer 1.3.2, where the Forms version is 4.5.7.1.2 (or 4.5.7.1.6 if you roll-on some patches).

Note: if you use NT 3.51, there is no problem with 1.3.

This often seen confusion arose due to the requirements statements on the product package and in the release notes stating that it would work with NT 3.51 or higher (.. meaning >= 3.51 but still < 4.0 !!)
