From my experience, FORMS45_PATH's list of directories is searched to find a menu module and any forms that may be used to satisfy a reusable-object reference.
The ORAPLSQLLOADPATH 's list of directories is searched to find a PLL or PLX file defined as an attached library within a form.
The ORACLE_PATH's list of directories is searched when the previous two variables did not yield a result.
