There is a convert45 utility which is supplied with forms 4.5 which can convert forms 3.0 application (.inp file ) to (.fmb) file.
Copy your good old forms 3.0 application (.inp) file to DOS. Run convert45 utility from $ORACLE_HOME\BIN
