Creating a Trouble Ticket

i´m just stucking in creating a troubleticket and dispatch db. What our copany wants to implement is every time a customer reports a new trouble a record in the following format should be created:

SA (always SA); Year (eg 03); Moth (eg: 01); Day (eg 23); runníng Number from 000 to 999

--> SA030123003

Any idea how to implement the running number or the service request? Thanks in advance.
