XML vs. databases

As more and more people demand XML being used in their database
projects, there are a few things I don't get. How do you delete or
update using XML?
Inserts I understand, updates can be done if every XML "record"
contains a unique ID - but delete? Do you have to include a delete
action as an element in the XML doc?
What are some other problems you have seen when integrating databases
and XML?
