They can be the same guy ;-) Well, i've seen things like CTO (who has things like installation, it and networking under him) who had no idea of tcp/ip nor routing desiging company global network, we fixed that for next year with bubblegum to make it work ... ;-) Usually PM's (i.e. development coordinators, usually just B.sc. or less) are common programmers around here, (in UK, similar could be lead programmers) and then development has a head that ties everything together that's like development manager or similar to dev (usually guy with atleast m.Ba or m.Business science degree whatever..) who has more teoretichal knowledge of software production chain and i responsible straight to board and other white-collar guys (and has things like research and dev under him). Database management are usually under it's own branch, or has it's leaf wiggiling under CTO with other infrastructure guys. Designers are placed into it's own branch under dev usually with no specific coordinator or manager.

(development) Managers do not code, they are like staff sargeants - in case of problems, making right people to talk to eachother. Manager has to have knowledge of basic things, so he can judge it as bad or worse in case of need. He needs the basic knowledge of whitebox/blackbox testing (he plans testing schedules with programmers and research), ER/UML models from designers and stuff like that because he is the one that ties the dept together. He's the guy who makes decisions in weekly meetings, listens the staff and acts accordingly. Also protects his staff from brown stuff coming from above (as in army they say 'shit always drains downwards') in case of need.
