Take a look at the technet site. Under the Internet Tools -> Developer 6.0 -> Documentation, you will find a pdf file detailing the steps involved in converting from forms 3.0 to 4.5/5.x/6.x. These instructions only work if you have your form definitions stored in the the database. It does not seem possible to be able to read the .INP format of the version 3 form.

If anyone has information on loading forms 3 INP files to the database for use in conversion, I would really like to learn how you did it. I still have not found a way. I no longer have the version 3 tools and when I did have access to them, they on on VAXvms.
