Use the ORACLE convert utility that comes with Forms 4.5. Once the forms are converted to 4.5 you can then progress to 5.0 and beyond.
You can find alot of whitepapers and info on the ORACLE site. It takes alot of work to convert forms from 3.0 to 4.5 .
We started the project of converting 200 complicated forms on may. Although the project is completed and the new Forms 5.0 are in production we still encounter problems. I assure you that we did alot of comprehensive testing.
I suggest you go move from 3.0 -> 4.5 -> 5.0. Then modify the forms and test each form after conversion. Try to use the new shared components that are available with Forms 5.0.
