I've had the same type of problem (bugs 555098/562865) with Forms 4.5 (Dev 1.3.2, 1.4W, 1.5.1) on Windows NT. Oracle assured me that it was fixed in version 2.0, but it might not have been fixed in time to make it into the initial production release. I'd be interested in hearing if anyone else is having similar problems in 4.5.
