Well, I'm not sure how elegant it is, but the most straight-forward approach I can think of is to put your address type list on a separate block from the multiple addresses. Then keep a hidden field on the block with the addresses indicating what type that address is. When they change type, search the addresses block for the correct address, and add it if not found.
