Just had the IT auditors in and on this subject two points came up:

  1. New developments must go through a User Acceptance Test before being released. Bug fixes can be released, but must be noted. A log table into which everyone puts 1 line for updates / bug fixesis fine.
  2. Not matter what you do, there must be a consistent and written methodology for planning . This holds for 1 developer or 100.
