It's not obvious, but you'll basically need to install the OMS software on the new server, then backup your existing OMS and restore it on the new one. At least, that's what I'm planning to do for 12.1.0.5 when I move from OL5 to OL7. Since I have a simple installation, I'll be using the scenario described in the above doc under "19.6.4.2 Single OMS, No SLB, OMS Restored on a Different Host".
