This would mean if no one has changed any configuration then you have SAN issues, from our experiences it is possibly a flaky HBA card but could be other issues with your SAN

That basically means what ASM can't reassemble the disk group since some of the disk(-) are missing.

First check the permissions and make sure what disk is actually exists and matches your asm_disk_string parameter.
